What is desquamation?

Desquamation is the normal shedding of dead corneocytes from the outermost layer of the skin. It is one stage of cell turnover — the tail end of the renewal cycle, where cells that have finished their journey finally release from the surface. Because it describes that quiet, continuous release, desquamation is also called skin shedding, corneocyte shedding, or natural sloughing, and the four terms all point to the same everyday event: spent surface cells detaching and drifting away so newer ones can take their place. It happens invisibly, one cell at a time, across the whole surface of healthy skin. The desquamation process has three moving parts: corneocyte release, enzyme cleavage, and surface flaking. Corneocytes are the flat, keratin-filled dead cells that make up the very top of the skin. They are held together by protein rivets called corneodesmosomes, and shedding cannot happen until those rivets are cut. A 2013 review described how keratinocytes reach this stage through cell death by cornification, maturing into corneocytes before they are shed. A 2000 study of corneocyte desquamation detailed how those dead cells detach from the surface as new cells rise beneath them, keeping the outer layer at a steady thickness.

Shedding rate, corneocyte cohesion, and enzyme dependence

Three attributes govern desquamation: the shedding rate, how tightly corneocytes cohere, and its dependence on enzymes. The shedding rate is how quickly surface cells release — fast enough and the skin stays smooth, slow enough and cells linger. Corneocyte cohesion is the counterforce: the strength of the bonds holding cells together. Shedding is really a balance between how firmly cells stick and how effectively that grip is loosened. Enzyme dependence is the mechanism that tips that balance. Desquamation relies on the skin’s own proteolytic enzymes to cut the protein bonds between corneocytes. A 1996 study identified the stratum corneum chymotryptic enzyme as a driver of corneocyte shedding, cleaving the connections so cells can detach on schedule. When shedding stalls: retention and buildup

The antonym of healthy desquamation is retention — cells that should shed but instead cling on. When corneocyte cohesion stays too strong or the shedding enzymes fall short, dead cells are retained at the surface rather than sloughing off. This is sometimes described as retention hyperkeratosis, a thickening of the surface from cells that overstay. Is desquamation the same as exfoliation?

Not quite. Desquamation is the skin’s own natural shedding, driven from within by enzymes that cut the bonds between dead cells.
