# Stratum Corneum: The Skin's Outermost Barrier Layer

> The stratum corneum is the top layer of the epidermis, a brick-and-mortar wall of corneocytes and lipids that holds water in and keeps irritants out.

## The skin's outermost layer

The stratum corneum is the outermost layer of the epidermis — the thin, resilient sheet that stands between the living skin beneath and the outside world. Though it is only tens of micrometers thick, it does the front-line work of the skin barrier: holding water inside the body and keeping irritants, allergens, and microbes out. The classic way to picture it is a brick wall. The bricks are corneocytes, flattened skin cells that have lost their nuclei and filled with a tough protein called keratin. The mortar between them is a matrix of specialized lipids. Together, this brick-and-mortar arrangement makes the layer both sturdy and nearly waterproof, and it is why an [indispensable barrier](https://pubmed.ncbi.nlm.nih.gov/19043850/) sits at the very surface of the skin rather than deeper down.

What makes the mortar work is its chemistry. The intercellular lipids of the corneum are dominated by ceramides, cholesterol, and free fatty acids, organized into ordered layers that repel water. When the lipid matrix is well-organized, the layer is strong and moisture stays in. When it is disorganized or depleted — by harsh cleansing, cold dry air, or a skin condition — the mortar thins, the wall leaks, and the surface dries out and turns reactive.

## How the corneum keeps skin comfortable

Because the stratum corneum controls how much water the skin holds, its condition largely determines whether skin feels supple or tight. A healthy corneum keeps its own water content high, which in turn keeps the layer flexible and its enzymes working properly, so old cells shed cleanly and the surface stays smooth. When the layer dries and its structure stiffens, that shedding falters, and skin looks flaky and feels rough. The barrier and the water it holds are two sides of one system.

### How thick is the stratum corneum?

It varies by body site. On most of the body it is only about 10 to 20 micrometers thick — a fraction of a millimeter — while on the palms and soles it is much thicker to withstand friction. On thin, delicate areas such as external vulvar skin it is especially fine, which is one reason those areas dry out and feel sensitive more easily and benefit from gentle, moisture-supporting care.

### What damages the stratum corneum?

Harsh soaps and surfactants strip its lipids, hot water and over-washing dry it out, and cold, low-humidity air pulls water from it faster than it can recover. Friction, over-exfoliation, and certain skin conditions also disrupt the brick-and-mortar structure. Once the mortar is depleted the layer leaks water and lets irritants in, which is why gentle cleansing and regular moisturizing matter.

### Can the stratum corneum repair itself?

Yes. The skin continually renews its outermost layer, pushing new corneocytes up from below to replace those that shed. When the barrier is disrupted, that repair machinery ramps up on its own, and supportive moisturizing gives it the water and lipid-like material it needs to rebuild more comfortably. Very compromised or persistently irritated skin, though, may need a clinician's guidance.
